 Client Application

I need an assistance dog, so what do I do now?
Receiving a service dog is a life changing event. In order to ensure that you will be placed with a companion that will best fit your needs and personality, we require some information to help us make that match a success. We know at times the application process may seem tedious, but rest assured the extra effort will pay off with a service dog that has been trained to empower you for independence!

To receive an application, please send the following information via post office mail:
1. A written request for a dog - Please include your phone number and email address.
2. A short autobiography
3. Tell us how you feel an assistance dog would benefit your life
4. A $50.00 non-refundable application fee
When we receive this information, we will mail you an application packet. Our current waiting list, after the receipt of a completed application packet, is 6 months to 2 years, depending on your needs and social styles.

Do I qualify for a Power Paws Assistance Dog?
Power Paws does not place dogs with children with Autism (case by case basis), seizure alert or response dogs, or those with multiple disabilities such as blind and deaf or wheelchair user and blind. This being said we will review each case individually.

Is there a charge for an Assistance Dog through Power Paws?
An approximate cost and value of a service dog when fully trained is $15,000 but because of donations we are able to place the dog for a minimal $6,000 fee for placements in Arizona, $7,000 for placements out of state. This fee can be raised by the individual through grants, sponsorships and other such programs such as The Assistance Dog United Campaign.

We look forward to working with you!
